OSH ACT of 1970 Section (5)(a)(1):Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards:  On or about May 9, 2020 employees engaged in well servicing  at the Well #DJ WCI 5H were exposed to struck-by hazards object hazards in that:  a). The employer failed to ensure vehicles and equipment were outside the fall lane of the derrick or mast, of the well servicing Rig No. 5 of Lone Star Well Services, when stability assembly components failed.  b). The employer failed to ensure that the base beam used in operation received approval by the manufacture or preform base beam capacity calculations prior to its use.  c). The employer failed to ensure that the wire rope used to guy Rig No. 5 was manufacturer's recommended minimum size and manufacturers recommended guying pattern.  d). The employer failed to communicate prior to commencing simultaneous operations any constraints, limitations, or conflicting activities.
