29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:  a) Worksite was located at 220 4th Avenue, Asbury Park, NJ 07712  Workers were exposed to struck by hazards as they were not wearing hard hats while cleaning up the debris on the ground from workers over head on the roof that were throwing down more removed roofing materials.  Violation was observed on or about May 17, 2022.

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Worksite was located at 220 4th Street, Asbury Park, NJ 07712  Employees were exposed to struck by hazards as they were tearing off a multifamily residential roof off without wearing safety glasses to protect their eyes from blowing debris.  Violation was observed on or about May 17, 2022.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Work site was located at 220 4th Avenue, Asbury Park, NJ 07712  Employees were exposed to fall hazards of approximately 22 feet while tearing off an old roof on a two story building with no fall protection on to prevent them from fall to the asphalt and concrete below.  Violation was observed on or about May 17, 2022.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Worksite was located at 220 4th Avenue, Asbury Park, NJ 07712  Workers were exposed to fall hazards as extension ladders they were using to access and egress the roof they were working from were landed properly and did not extend three feet above their landing point as required.  Violation was observed on or about May 17, 2022.
