29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting respirator is fit tested prior to initial use, whenever a different respirator facepiece (size, style, make or model) is used, and at least annually thereafter. a) Hospital of the University of Pennsylvania - 501 S. 54th Street, Philadelphia, PA 19143, 3rd Floor: An employee required to wear filtering facepiece respirator was not fit tested prior to use. The employer required employees to wear N95 filtering facepiece respirators such as but not limited to 3M 8210 to protect against the SARS-CoV-2 virus while providing care to confirmed positive COVID-19 residents. Condition noted on/about May 18, 2022
