OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of  employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards from falling or collapsing wood trusses and walls.  Trainor Construction Inc. at the worksite located at 580 Derby Line Road Genoa, Illinois:   On May 17, 2022, employees installing truss spacers and nailing truss heels to wall top plates were exposed to struck-by hazards when working on and between trusses that had not been braced to resist out-of-plane buckling, leaning, and/or collapse due to the lack of top cord, web member, bottom chord and diagonal bracing.

29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about May 17, 2022 employees were not protected by hard hats while working around loads (walls) suspended by a crane, exposing employees to struck- by hazards.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Instance a. On or about May 12, 2022, employees were not protected from a fall while sheeting a second-floor deck at a craft barn, exposing employees to a fall of approximately 10 feet.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about May 17, 2022, the employer did not verify compliance by preparing written certification of employees fall protection training, exposing employees to a falls up to 18 feet.
