29 CFR  1910.134(f)(2):The employer did not ensure that employees using a tight-fitting respirator were fit tested prior to initial use of the respirator, whenever a different respirator facepiece was used, and at least annually thereafter:  Facility at 7 Sandisfield Road, Sandisfield, MA: On or about May 25, 2022, and at times prior, the employer did not provide a fit test to all employees required to wear a respirator, prior to initial use, and when a different respirator was used.  Employees were required to wear N95 filtering-facepiece respirators to protect against SARS-CoV-2 virus while providing care, such care as feeding, washing, dressing, and other personal care, to suspected and confirmed COVID-19 positive residents.
