29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  a) On or about 5/22/2022, in the Molding Department, employees were exposed to crush between hazards when working around the mold transfer shuttle. Employee's head got crushed between the mold transfer shuttle and support I beam when the shuttle was moving away from the machine as the employee was walking to the other side of the mold transfer shuttle.  b) On or about 5/25/2022, in the Molding Department, employees were exposed to crush between hazards when working around the mold transfer shuttle. Employees are exposed to crush between hazards when the shuttle moves towards the machine, as it creates a pinch point between the I beam and the rear of the mold transfer shuttle.  ABATEMENT DOCUMENTATION REQUIRED
