(b) Northwest end of excavation; A four-sided Arcosa Shoring Products brand Build-A-Box Modular Trench Shielding System was not used in accordance with the manufacturer's tabulated data in that backfilling was not completed and/or sufficient around all sides to provide a 6-inch or less clearance between the shielding system and excavation walls.
