29 CFR  1910.132(d)(1)(i): The employer shall select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment.  At the facility located at 2955 Ramada Way, Green Bay, WI 54304, the employer did not select and require the use of personal protective equipment that will protect employees from the hazards identified in the hazard assessment while recycling materials, such as but not limited to fluorescent lamps, exposing employees to the hazards associated with mercury (aryl and inorganic). Employees' skin and hair had repeated contact with accumulations of mercury dust while working in the Bulb Room.

29 CFR  1910.333(a)(1): Live parts to which an employee may be exposed shall be deenergized before the employee works on or near them, unless the employer can demonstrate that deenergizing introduces additional or increased hazards or is infeasible due to equipment design or operational limitations. Live parts that operate at less than 50 volts to ground need not be deenergized if there will be no increased exposure to electrical burns or to explosion due to electric arcs.  On or about July 22, 2022, the employer did not ensure that live parts to which an employee was exposed to were deenergized before working on them, including tasks such as but not limited to resetting the contacts inside the electrical panel for the conveyor, bulb crusher, and filter system, exposing the employee to electrical shock hazards.
