29 CFR 1910.28(b)(1)(i):(1) Unprotected sides and edges. (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling. At this location, employees climb on top of a concrete truck so that material can be loaded into the tank of the truck. The height exceeds 10 feet and this exposes employee to fall hazards.

1910.303(g)(1)(ii) Working space required by this standard may not be used for storage. When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded. 29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. On or about June1, 2022, at this location, clutter was in front of the electrical panel boxes. This exposed employees to electrical hazards.
