29 CFR  1910.134(e)(6)(i): The employer did not obtain from the physician or other licensed health care professional (PLHCP)  a written recommendation regarding the employee's ability to use the respirator:    Lewis Memorial Christian Village located at 3400 W Washington in Springfield, IL   On or about June 2, 2022, the employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus and the employer did not obtain a written recommendation regarding the employee's ability to use the respirator prior to the fit test.

29 CFR  1910.502(q)(2)(ii)(A):The COVID-19 log did not contain, the employee's name, one form of contact information, occupation, location where the employee worked, the date of the employee's last day at the workplace, the date of the positive test for, or diagnosis of, COVID-19, and the date the employee first had one or more COVID-19 symptoms, if any were experienced.    Lewis Memorial Christian Village located at 3400 W Washington in Springfield, IL.  On or about June 2, 2022, the employer failed to ensure the COVID-19 log contained at least one form of contact information, the date of the employee's last day at the workplace and the date the employee first had one or more COVID-19 symptoms, if any were experienced.
