29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  On or about June 10, 2022, and at times prior thereto, the employer did not perform frequent and thorough inspections of the jobsite while workers were framing a Senior Residential Complex at heights above approximately 8 feet, exposing employees to work related construction hazards.

29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:   On or about June 10, 2022, and at times prior thereto, an employee was walking across second story ceiling joists while installing trusses for a Senior Residence complex at heights above 8 feet without being properly tied off, exposing employees to a fall hazard.
