29 CFR  1904.32(a):  The employer did not ensure that at the end of the year that entries were complete and accurate; and an annual summary of injuries and illnesses on the OSHA 300 Log was created and certified.   a) Bear Valley Golf Course - On or about June 16, 2022, a senior establishment management official did not ensure illness and injury entries were complete and accurate, AND an annual summary of injuries and illnesses on the OSHA  300 logs was not certified and posted for calendar year 2021 resulting in materially impairing the understandability of the injuries and illnesses in the workplace.   b) Bear Valley Golf Course - On or about June 16, 2022, a senior establishment management official did not ensure illness and injury entries were complete and accurate, AND an annual summary of injuries and illnesses on the OSHA  300 logs was not certified and posted for calendar year 2020 resulting in materially impairing the understandability of the injuries and illnesses in the workplace.   c) Bear Valley Golf Course - On or about June 16, 2022, a senior establishment management official did not ensure illness and injury entries were complete and accurate, AND an annual summary of injuries and illnesses on the OSHA  300 logs was not certified and posted for calendar year 2019 resulting in materially impairing the understandability of the injuries and illnesses in the workplace.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) Maintenance Shop - On or about June 16, 2022,  an emergency eye wash, model: Porta Stream I, serial number: 263768, manufactured by Honeywell to flush eyes in the event of an emergency was not suitable in that it was not inspected at least monthly and maintained in accordance with manufacturer's instructions, exposing employees to biological hazards.
