29 CFR 1917.118(f)(2):   Ladders consisting of individual rungs attached to walls, conical manhole sections or river cells did not form a continuous ladder, uniformly spaced vertically from 12 inches to 16 inches (30.5 to 40.6 cm) apart with a minimum width of 10 inches (25.4 cm) and projecting at least 4�? inches(11.43 cm) from the wall.   On or about June 22, 2022, on the slack water bay dock, employees climbed the fixed ladder to access barges.  The rungs of the ladder were not uniformly spaced vertically.

29 CFR  1917.118(c)(1): Ladders with broken, split or missing rungs, steps or rails, broken welds on connections, corrosion or wastage, or other defect which may have affected safe use were not removed from service:  On or about June 22, 2022, on the slack water bay dock, employees climbed a fixed ladder to access barges.  The fixed ladder had deficiencies including but not limited to the following:  (a)  The top rung was missing;  (b)  Existing rungs were bent and/or otherwise deviated from center.
