29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   On or about June 29, 2022 and times prior to, the employer did not establish specific procedures for energy control,  to protect employees from unexpected releases of energy while performing maintenance and troubleshooting operations on the following machines:  (a) 600 ton press in the Brake and Laser Building. (b) 650 ton press in the Brake and Laser Building. (c) 250 ton press in the Brake and Laser Building. (d) 350 ton press in the Brake and Laser Building. (e) 1000 ton press in the Brake and Laser Building. (f) 230 ton press in the Brake and Laser Building.

29 CFR  1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:   On or about June 29, 2022 and at times prior thereto, maintenance employees were exposed to electric shock of 240 volts due to a missing knockout in the circuit-breaker panel #10 in the Brake and Laser Building.
