On or about July 5, 2022, at the above addressed jobsite, an employee performing residential roofing operations, ten (10) feet one (1) inch above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  b. On or about July 5, 2022, at the above addressed jobsite, an employee performing residential roofing operations, ten (10) feet one (1) inch above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  c. On or about July 5, 2022, at the above addressed jobsite, employees performing residential roofing operations, eleven (11) feet four (4) inch above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  The Jose Rodea dba Jose Rodea was previously cited for a violation of this occupational safety and health standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1536829, citation number 1, item number 1 and was affirmed as a final order on November 10, 2021, with respect to a workplace located at 437 Brassie Lane, University Park, IL 60484.

On or about July 5, 2022, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
