Chevron Environmental Management Company, Wilson Creek Unit 69, Rig #5545 2500 County Road 9 Meeker, CO 81641: On June 29, 2022, the controlling   employer did not ensure that the rig floor of Ranger Energy Services Rig #5545 was kept in a clean and sanitary condition. The employer was supervising a stripping job being performed on a well that contained a continuous rod. To perform this stripping job, the rig crew was pulling wet strings of tubing from the well without implementing a way of conveying the fluids away from the rig floor. The rig floor was covered in wax paraffin, oil, and flammable liquids. This condition exposed employees to slip, trip, and fall hazards.
