29 CFR  1910.147(a)(3)(i):The employer did not utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices, and to otherwise disable machines or equipment to prevent unexpected energization, start up or release of stored energy in order to prevent injury to employees.  a) At Dolomite Products Company's asphalt plant in Walworth, on or about 7/11/2022  LOTO  procedures were not utilized. The employer failed to ensure an employee applied lockout and/or tagout devices to the conveyor belt, while performing maintenance, removing a stone clump clog, the conveyor was restarted by another employee, injuring the employee clearing the stone clump clog.
