29 CFR 1910.29(b)(3): The employer did not ensure that guardrail systems are capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in a downward or outward direction within 2 inches (5 cm) of the top edge, at any point along the top rail. a) Post Printing CO. on or about June 27th, at the address of 205 West 4th Street Minster, Ohio: The employer did not ensure the quarter inch thick plywood material utilized as a guard across the opening of the elevator shaft was of adequate strength. An employee performing maintenance activities adjacent to the inadequately guarded elevator shaft was exposed to a fall of 13ft. to the level below.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Post Printing CO. on or about June 27th, 2022, at the address of 205 West 4th Street Minster, Ohio: The employer failed to report an employee hospitalization (admittance) following a workplace incident (fall from a height). The employer or employer representative failed to contact the Toledo Area Office or the 24 hour OSHA number to notify the agency of employees' hospitalization within 24 hours of admittance.
