29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   On or about July 21, 2022, employer did not ensure that eye protection was worn while employees were utilizing air powered nail guns during construction activities on a residential home located at 3961 Summerfield Lane Lot #24 Richfield, Ohio 44286.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about the July 21, 2022, employees engaged in residential roofing  construction activities atop a roof located at 3961 Summerfield Lane Lot #24 Richfield, Ohio 44286, 18.6  feet above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection by employer.  Stoney Acres Builders, LLC,  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number, citation number 1432743, citation number 2 item number 1  and was affirmed as a final order on 10-23-2019 , with respect to a workplace located at 189 Northland Road Medina, Ohio 44256.  Stoney Acres Builders, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1262122, citation number 1, item number 1 and was affirmed as a final order on 11-03-2017, with respect to a workplace located at 6950 Kingswood Hudson, Ohio 44236.
