Location: Home Depot, 22 Ashbrook Rd. in Keene, NH - On August 5, 2022, the employer failed to provide an OSHA representative OSHA 300 logs and 300A summary forms for 2019, 2020 and 2021 within four business hours of being requested. The request was made at 10:45AM on August 5, 2022 and documents were not received from the employer until August 30, 2022.

29 CFR  1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. Your establishment had 20 or more employees but fewer than 250 employees during the previous calendar year, and your establishment was classified in an industry listed in appendix A to subpart E of this part, and the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. Submission for this information was required once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:  Located at the Home Depot, 22 Ashbrook Rd. in Keene, NH: as of July 28, 2022, the employer failed during calendar year 2021 to electronically submit information from their OSHA Form 300A or equivalent by March 2, 2022.
