BASKETBALL GYMNASIUM: On or about August 2, 2022, the employer had not conducted inspections on the Tallescope Upright scaffold prior to an employee using it to perform vent cleaning with a vacuum resulting in a fatal fall.

BASKETBALL GYMNASIUM: On or about August 2, 2022, the employer did not ensure that the Tallescope scaffold was equipped with outrigger beams to prevent any movement during use and that the locking hooks at the base of the ladder were securely locked in place to prevent tipping at the fulcrum point.
