29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) Townhouse Apartments, Clarkson University, 8 Clarkson Avenue, Potsdam, NY  13699, on or about 7/20/22:  Employees were using an aerial lift to access and egress upper portions of the main roof in excess of 20 feet from ground, without being attached to the boom or basket.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Townhouse Apartments, Clarkson University, 8 Clarkson Avenue, Potsdam, NY  13699, on or about 7/20/22:  An employee was performing roofing work on a 5:12 pitch roof, with a ground to eave height of 9 feet, without fall protection of any kind.    b) Townhouse Apartments, Clarkson University, 8 Clarkson Avenue, Potsdam, NY  13699, on or about 7/20/22:  Employees were performing roofing work on a 5:12 pitch roof and 8:12 pitch dormers, with a ground to eave height of 20 feet, without fall protection of any kind.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) Townhouse Apartments, Clarkson University, 8 Clarkson Avenue, Potsdam, NY  13699, on or about 7/20/22:  An employee was performing roofing work on a 5:12 pitch roof, with a ground to eave height of 9 feet, and used a portable ladder to access/egress that roof without that ladder extending at least three feet above the upper landing.
