29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   (a) 10642 S. Susquehanna Trail, Liverpool, PA - On or about August 16, 2022, the cage used by employees to clean the snow off the top of modular homes was not approved for use by the manufacturer of the powered industrial truck(s) as an attachment.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:   (a) 10642 S. Susquehanna Trail, Liverpool, PA - On or about August 16, 2022, the powered industrial truck was not marked to identify and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered for the cage attachment used for snow cleaning operations.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
