29 CFR 1910.151(b):  Adequate first aid supplies were not readily available.  a) On or about July 21,2022, an automated external defibrillator (AED) had pads that expired September 2021, exposing employees to potential shock.  b) On or about July 21, 2022, Maximum Strength Non Aspirin Pain Reliever/Fever Reducer was stored in the first aid kit for employee use, but they expired on May 2017, exposing employees to biological hazards.  c) On or about July 21, 2022,  Aspirin Pain Reliever, manufactured by Heart Heath was stored in the first aid kit for employee use, but they expired on November 2017, exposing employees to biological hazards.  d) On or about July 21, 2022, Triple Antibiotic Ointment was stored in the first aid kit for employee use, but it expired on July 2018, exposing employees to biological hazards.

29 CFR 1960.67 As required by 29 CFR 1904.32, the Agency did not certify that he or she has examined the OSHA 300 log and that the annual summary was correct and complete:  29 CFR 1904.33(a) The employer did not save the OSHA 300 Log, the privacy case list (if one existed), the annual summary, and the OSHA 301 Incident Report Forms for five (5) years following the end of the calendar year that these records covered.   a) On or about July 21, 2022, the employer did not retain at the establishment OSHA 300 logs for fiscal years 2019, 2020, 2021, and 2022.  b) On or about July 21, 2022. the employer did not retain at the establishment OSHA 300A annual summary forms for fiscal years 2019.
