29 CFR 1910.147(d): Application of control.  The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about July 22, 2022, at a workplace located at 747 Bissell Street, White Lake, WI 54491; employees were exposed to machine hazards associated with moving parts when they engaged in unjamming activities on the Old Glue Press tunnel.  The employer did not ensure that energy control application steps to control the electrical energy sources were implemented prior to allowing an employee to remove a jam on the Old Glue Press tunnel.  The machine was not locked out prior to engaging in this servicing and maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.
