OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees  in that employees were exposed to fall, struck-by and crushed-by hazards associated with the inadequate storage and installation of prefabricated roof trusses:  a.  On or about August 17, 2022, at the Amenity building construction site in Tallahassee, Florida: The employer exposed employees to stuck-by and crush-by hazard, in that roof trusses were not stored, handled and installed in accordance with the manufacturer's instructions to prevent lateral bending and structural damage.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   a. On or about August 17, 2022, at the Amenity building construction site in Tallahassee, Florida: during roof truss erection, the crew standing on the top wall or on roof truss components such as, but not limited to web members and bottom chord of the trusses, were not protected from a 12-foot fall hazard by the use of a fall protection system.

29 CFR  1926.1428(a): The employer of the signal person did not ensure that each signal person met the Qualifications Requirements (paragraph (c) of this section) prior to giving any signals:   a. On or about August 17, 2022, at the Amenity building construction site in Tallahassee, Florida: during the lifting and hoisting of 45-foot-long span roof trusses, the employer exposed employees to a struck-by hazard, in that; an employee performing hand signal activities was not qualified for the task.
