29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. On a residential roof, at a temporary worksite located at 3006 Thorn Glen Court in Jacksonville, Florida 32208: On or about August 17, 2022, the employer exposed employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using pneumatic nail guns to install roofing shingles.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a.  On a residential 5:12 pitched roof, at a temporary worksite located at 3006 Thorn Glen Court in Jacksonville, Florida 32208: On or about August 17, 2022, the employer exposed seven employees to fall hazards ranging from 6-foot to 21-foot, in that fall protection was not utilized by employees while installing roof shingles on the roof.
