The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) Signature Cabinetry located at 1285 Alum Creek Drive, COLUMBUS, OH 43209: On and preceding August 23, 2022, the employer did not establish an energy control program which established energy control procedures, employee training and periodic inspections when employees were engaged in tooling changes on machinery which included but was not limited to a GMM CNC Stone Saw. Employees perform material exchanges, periodic maintenance, cleaning and blade changes on a GMM Extra CNC stone saw. A hazardous energy control program was not developed and implemented to prevent unexpected energizing, startup or release of stored energy thereby exposing employees to caught-in and struck-by hazards.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Signature Cabinetry located at 1285 Alum Creek Drive, COLUMBUS, OH 43209: On and preceding August 23, 2022, employees were exposed to machine hazards associated with moving parts during tooling changes and servicing on machinery with multiple energy sources. Employees perform blade changes and periodic maintenance on a GMM Extra CNC stone saw. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect and all other energy sources [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(S)(i)J, and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. The following condition exposed employees to caught-in and struck-by hazards.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) Signature Cabinetry located at 1285 Alum Creek Drive, COLUMBUS, OH 43209: On and preceding August 23, 2022, employees were exposed to machine hazards associated with moving parts, scattering fragments and an 18 inch circular saw blade point of operation during stone cutting cycles on a GMM Extra CNC stone saw. Operators and other employees perform work in the slab cutting area where granite/quartz slabs are cut which includes monitoring of the cuts and material exchanges are performed on the GMM Extra CNC stone saw. The folding barrier was not being utilized on the front side of the CNC saw to prevent scattering fragments of material from striking employees and to prevent entry into the machine area during operation. Additionally, no operational presence sensing devices were installed on the machine to cease cutting in the event the machine area were entered for minor adjustments or to remove scrap material during operation. The following condition exposed employees to caught-in and struck-by hazards.
