29 CFR  1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage:  a) Between Buildings 700/G and 800/H: On or about August 29, 2022 and at times prior thereto, the controlling employer, Gunner/Houston, LTD, Inc. failed to ensure that its subcontractor MBA Installers, Inc. protected its employees from electrical shock hazards in that an extension cord, powering a hammer drill that the employee was using, was run across an asphalt street, exposing it to damage by vehicular traffic.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Building 800/H, 1900 Elevate Avenue Project: On or about August 29, 2022 and at times prior thereto, the controlling employer, Gunner/Houston, LTD, Inc. failed to ensure that its subcontractor Jibram Contractors, LLC protected its employees from fall hazards while performing framing activities at heights up to 11 feet 5 inches to the ground below.  Gunner Houston, LTD, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.501(b)(13), which was contained in OSHA inspection number1364825, Citation 1, Item 1 and was affirmed as a final order on April 08,2019,  with respect to a workplace located at 1300 County Road 210 in St John, Florida.
