29 CFR  1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made:  a) On September 1, 2022 and times prior, safe clearances were not designated between employees and the powered industrial trucks used to handle various sized bundles of rebar in the rebar fabrication and storage yard.  Traffic control signs were not used, or a traffic pattern established, potentially exposing employees to a struck-by hazard from forklifts and other vehicles. 29 CFR  1910.176(b): Storage of material created a hazard:   a) On September 1, 2022, and times prior industrial rack systems used to store scrap rebar were damaged.  This included but may not be limited to, rack arms and intermediate braces that were bent and/or slanted downward.  Rebar bundles were not secured against sliding off the rack arms.  Rebar bundles were not positioned as close to the rack uprights as possible.  Stickers placed between rebar bundles were not secured.
