29 CFR  1910.28(b)(1)(i):The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  Dock, gangway:  Numerous sections of standard guardrails were missing  from the edge of the dock and at the top of the gangway.  Employees work over water and near the edge of the dock and are exposed to fall hazards of more than 4 feet into the bay.

29 CFR  1915.55(a)(1):Valve protection caps on gas Welding and cutting cylinders were not in place or secure.  Dock:  An oxygen cylinder stored in a cart alongside a cylinder of acetylene did not have a valve cap in place when not in use.
