29 CFR  1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  a) At GT Harvesting located at 1126 Raymond D Bland Rd, Glennville, GA, 30427: on or about September 15, 2022, and at times prior to, the employer exposed employees to struck by hazards in that each forklift operator working in the sweet potato farm did not received an evaluation of their performance at least every 3 years.

Abatement Certification and Documentation is required.  a) At GT Harvesting located at 16556 GA Hwy 23, Reidsville, GA 30453: on or about September 15, 2022, and at times prior to, the employer failed to keep and update their OSHA 300 and 300A logs, summaries of work-related illnesses in that the company employed 130 employees.
