Location: 109 Chase Rd, Marlborough, MA  On or about 9/15/22, employees assigned to the jobsite were not trained in the recognition of fall hazards.

Location: 109 Chase Rd, Marlborough, MA  On or about 9/15/22, employees were exposed to fall hazards while climbing and descending portable ladders that did not extend at least 3 feet above the upper landing surface (roof).
