29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a) New commercial construction located at 4855 Windward Parkway, Milton, GA 30004, Roof: On or about 09/16/2022, the employer exposed employees to struck-by hazards while operating Metabo Framing nail guns, in that protective eyewear was not provided to employees operating these tools at 120 PSI firing 2-3/8-inch and 3-inch plastic collated fasteners engaged in framing and sheathing activities.

29 CFR  1926.502(d)(6)(i): A snap hook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snap hook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snap hook such that unintentional disengagement could occur by the connected object being able to depress the snap hook keeper and release itself.   a) New commercial construction located at 4855 Windward Parkway, Milton, GA 30004, Roof: On or about 09/16/2022, the employer exposed employees to fall hazards of 12-feet to the lower ground level, in that personal fall arrest systems was not anchored to the metal anchor provided with the fall protection system as employees used the snap hook of the lifeline and tied it around the roof truss connecting back to the lifeline (rope) which is not recommended by the manufacturer of the personal fall protection system.

29 CFR  1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.   a) New commercial construction located at 4855 Windward Parkway, Milton, GA 30004, Roof: On or about 09/16/2022, the employer exposed employees to fall hazards of 12-feet, 7-inches to the lower concrete surface, in that employees were not provided with a ladder free from damage to the top side rails as they were bent or missing and did not extend at least 3-feet past the landing.
