On or about September 15 , 2022, at jobsite located in 459 Marion Ave, Aurora, IL 60505, an employee engaged in roofing activities was working below the other employees and exposed to struck by hazards from falling objects. Eye protection was not being utilized while employees engaged in this activity.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system:  On or about September 15, 2022, at jobsite located in 459 Marion Ave, Aurora, IL 60505, employees were exposed to fall hazards in excess of 18 feet while engaged in residential roofing activities. Fall protection was not being utilized while performing roofing activities.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about September 15, 2022, at jobsite located in 459 Marion Ave, Aurora, IL 60505, the employer failed to ensure that employees had been trained to recognize and minimize fall hazards. Employees were exposed to fall hazards in excess of 18 feet while performing roofing activities on a residential structure without using fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about September 15, 2022 at jobsite located in 459 Marion Ave, Aurora, IL 60505 employees used an extension ladder that did not extend three feet past the roof they were accessing, exposing the employee to a fall of approximately 10 feet.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
