29 CFR  1926.100(a):At this job site, employees exposed to possible head injury from falling or flying objects or from impact with hand tools, equipment and building materials during construction operations were not wearing hard hats.   On or about September 20, 2022, and times prior to, one employee was working on the ground and passing material above his head to employees working on the roof without wearing a hard hat, exposing the employee on the ground to struck-by hazards.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about September 20, 2022, and times prior to, three employees were not using fall protection while performing roofing operations of a new residential home; exposing the employees to fall hazards.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about September 20, 2022, and times prior to, three employees were using a ladder to climb to the roof in which the side rails of the ladder did not extend three feet above the walking surface of the roof, exposing the employee to fall hazards.
