29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   On or about September 27, 2022, employees were exposed to a fall of 19.5 feet while performing roofing work on a commercial building located at 6715 Tippecanoe Road, CANFIELD, OH 44406. The employer did not ensure the employees working on all areas of the roofing were utilizing any form of fall protection.   Choice Roofing& Siding, LLC,  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1577610, citation number 1, item number 2 and was affirmed as a final order on 03/09/2022, with respect to a workplace located at 3994 Braemar, BROADVIEW HEIGHTS, OH 44147 .

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about September 27, 2022, employees were exposed to a fall of 19.5 feet while performing roofing activities on a commercial building located at 6715 Tippecanoe Road, CANFIELD, OH 44406. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended three feet above the roof landing.   Choice Roofing & Siding, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1577610, citation number 1, item number 3 and was affirmed as a final order on 03/09/2022, with respect to a workplace located at 3994 Braemar, Broadview Heights, Ohio 44147. .
