29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazards identified in the hazard assessment:  (a) On or about 09/16/2022 and at times prior to at 18060 AL Hwy 21, Sycamore, AL in Tapping, employees were exposed to amputation hazards while operating the Gamor Vitoria M3-M130 Vertical Hydraulic Tapping Machine because the employer provided the employees cloth cut resistant gloves which could be caught and wrapped around the unguarded rotating tap bit.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) On or about 09/16/2022 and at times prior to at 18060 AL Hwy 21, Sycamore, AL in Tapping, employees were exposed to amputation hazards while operating the Gamor Vitoria M3-M130 Vertical Hydraulic Tapping Machine because the tap was not guarded exposing operators' to the rotating tap bit.

29 CFR  1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone:  (a) On or about 09/16/2022 and at times prior to at 18060 AL Hwy 21, Sycamore, AL in Tapping, employees were exposed to amputation hazards while operating the Gamor Vitoria M3-M130 Vertical Hydraulic Tapping Machine because a tool was not available to move tapped nuts from the jig to the bin without the operator getting too close to the rotating tap bit.
