29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: a) On October 5, 2022, employees were exposed to the hazard of falling approximately 40 feet to the ground while performing carpentry work from an elevated work platform supported by the forks of a JLG Skytrak 10054 all-terrain forklift. The work platform was not attached to the forks of the all-terrain forklift. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level: a) On October 5, 2022, employees engaged in carpentry work were not protected from the hazards of falling approximately 40 feet to the ground while working from an elevated work platform supported by the forks of a JLG Skytrack 10054 all-terrain forklift. The work platform was not equipped with a standard guard rail system. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
