29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  11227 W. 1604 loop N., San Antonio, TX 78254: On or about October 07, 2022, and times prior thereto, employees were exposed to a fall hazard while installing drip edge flashing on a commercial roof building without any fall protection.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  11227 W. 1604 loop N., San Antonio, TX 78254: On or about October 07, 2022, and times prior thereto, employees were exposed to a fall hazard of greater than 6 feet, while working on the edge of commercial buildings roof, without being trained on fall hazards recognition.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  11227 W. 1604 loop N., San Antonio, TX 78254: On or about October 07, 2022, and times prior thereto, workers were exposed to fall hazards while using extension ladders with rails extended less than three feet, to access a rooftop to install drip edge flashing.
