29 CFR 1910.30(a)(1):(a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart. Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. On or about October 11, 2022, at a residence located at 1082 Olson Avenue Oshkosh, Wisconsin; the employer did not provide training on fall hazards in the work area and how to recognize them prior to an employee being exposed to fall hazards while working from an elevated basket to remove tree limbs at heights of approximately 35 feet.

On or about October 11, 2022, at a residence located at 1082 Olson Avenue Oshkosh, Wisconsin; the employer did not ensure that an employee in an aerial lift was wearing a personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I. The employee was working from an elevated basket to remove tree limbs at heights of approximately 35 feet.
