29 CFR  1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible.  (a) At the OMAL Dowel Machine #1, located at 755 Raco Drive, Lawrenceville, GA: On or about October 11, 2022, and at times prior, employees were exposed to an amputation hazard from moving parts, in that the employer did not ensure the machine guard was securely affixed to the machine, to prevent the guard from being adjusted or removed.

29 CFR  1910.212(a)(3)(iii): Special hand tools for placing and removing material were not provided to permit easy handling of material without the operator placing a hand in the danger zone:  (a) At the OMAL Dowel Machine #1, located at 755 Raco Drive, Lawrenceville, GA: On or about October 11, 2022, and at times prior, employees were exposed to an amputation hazard from moving parts, in that the employer did not provide special hand tools for placing and moving various sized wooden cabinet parts beyond the machine guard.

29 CFR  1910.242(b): Compressed air, that was not reduced to less than 30 p.s.i., was used for cleaning purposes without effective chip guarding and personal protective equipment.  (a) At the plant, located at 755 Raco Drive, Lawrenceville, GA: On or about October 11, 2022, and at times prior, employees were exposed to a struck-by and impalement hazard, in that employees used compressed air nozzles with air over 30 p.s.i. for cleaning purposes.
