29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used: a) Jobsite, at or near 4557 Ohio River Boulevard, Avalon, PA: On or about October 18, 2022, during electrical installation work, the employer did not ensure that a safe way of getting on and off of a pre-fabricated walk through type scaffold was used in that employees climbed the end frames and guardrails on the scaffold to get up/down, exposing them to a fall hazard of approximately 12 feet.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) Jobsite, at or near 4557 Ohio River Boulevard, Avalon, PA: On or about October 18, 2022, during electrical installation work, the employer did not ensure that appropriate fall protection systems/devices were in use along the ends of scaffolding in that there were no guardrails along the ends of the 2nd level of scaffolding, exposing employees to fall hazards of up to approximately 12 feet.
