29 CFR  1926.251(a)(4): Special custom design lifting accessories for material handling were not marked to indicate the safe working load(s):  a.   On or about August 17, 2022, at the Amenity building construction site in Tallahassee, Florida: the creating employer Forgotten Coast Crane Service, Inc. failed to ensure that the employees of Big Hammer, LLC were protected from struck-by hazards during the lifting and hoisting of 45-foot long span roof trusses, in that; the steel-rod custom made hooks had not been proof tested prior to use and marked with the rated load capacity.

29 CFR  1926.1419(f):  During operations requiring signals, the ability to transmit signals between the operator and signal person were not maintained, and the operator did not safely stop operations requiring signals until it was reestablished and a proper signal was given and understood:   a. On or about August 17, 2022, at the Amenity building construction site in Tallahassee, Florida: the creating employer Forgotten Coast Crane Service, Inc. failed to ensure that the employees of Big Hammer, LLC were protected from struck-by hazards during the lifting and hoisting of 45-foot long span roof trusses, in that; the crane operator did not maintain communication with the hand signal person, nor safely stopped the operation until hand signal communication was reestablished.
