29 CFR 1926.651(k)(1): An inspection was not conducted by the competent person to identify evidence of other hazardous conditions as needed throughout the shift. Location - 75 Middlesex Turnpike, Burlington, MA. On or about October 20, 2022, an excavator operator was permitted to maneuver an excavator with the bucket raised above the superstructure and in the direct vicinity of overhead communication lines. Overhead lines were struck, pulling down an adjacent utility pole with energized electrical lines.
