29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a.)  On or about October 15, 2022, in the Tunnel 2 Washroom, an employee sustained an amputation while inspecting the Braun Press Extractor 2, Serial No. BPE1302K6007R17.  The employer failed to implement energy control application steps as the machine was not shut down or turn off to perform the servicing work [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a.) On or about October 15, 2022, and dates prior to, in the Tunnel 2 Washroom, authorized employees performed service and/or maintenance on the Braun Extraction Press 2, Serial No. BPE1302K6007R17.  The employer did not perform a periodic inspection of the energy control procedure to ensure the procedure was being followed.  Five Star Laundry - Chicago, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 1521902, citation number 1, item number 1B and was affirmed as a final order on August 12, 2021, with respect to a workplace located at 1060 W. Division Street, Chicago, Illinois 60642.  Five Star Laundry was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 1394782, citation number 1, item number 3 and was affirmed as a final order on September 24, 2019, with respect to a workplace located at 6000 Osceola Parkway, Kissimmee, FL 34746.
