29 CFR  1910.303(b)(2):  Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling:  At a facility located at 1711 Scheuring Road, De Pere, WI 54115; the electrical equipment in the Abrasive Blast tent used to power the abrasive blasting pot equipment and compressed air dryer equipment were not used in accordance with instructions in the listing and labeling of the equipment. A relocatable power tap (RPTs or power strips) was connected in series ("daisy chained") with multiple extension cords to supply electrical power to equipment. The relocatable power tap was not used indoors and was used in a location for which it was not intended.

29 CFR  1910.305(g)(1)(iv)(A):  Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used as a substitute for the fixed wiring of a structure:   At a facility located at 1711 Scheuring Road, De Pere, WI 54115; flexible extension cords in the Abrasive Blast tent were used to power the abrasive blasting pot equipment and compressed air dryer equipment. The extension cords were zip tied to the tent's structural pols and were used as a substitute for fixed wiring.

29 CFR  1910.1200(h)(1):  Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets:    At a facility located at 1711 Scheuring Road, De Pere, WI 54115; the employer did not provide employees with effective information and training on the hazardous chemicals in their work area. Employees who spoke Spanish as their first language did not receive training in a language that they understood and were therefore not effectively trained in the requirements of this section.  All provisions of 29 CFR 1910.1200(h)(1) through (h)(3) must be covered in hazard communication training. Key elements include, but are not limited to the following:  1)  Requirements of this section 2)  Location of SDS and Hazard Communication Program 3)  Hazard of chemicals in work areas 4)  Measures to protect from hazards of chemicals
