Section 4.1.2(c) American National Standards Institute 01.1-1961, Safety Code for Woodworking Machinery as adopted by 29 CFR 1926.304(f): Circular hand-fed rip saws did not have non-kickback fingers on dogs so located as to oppose the tendency of the saw to pick up the material or to throw it back toward the operator:  Jobsite located at 131 Beacon Hill Drive, West Hartford, Connecticut:  On, or about, August 18, 2022, a DeWalt portable table saw, Model DWE7491, serial number 950776, was not provided with anti-kickback fingers.

29 CFR  1926.304(i)(1):  Each circular had-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut:  Jobsite located at 131 Beacon Hill Drive, West Hartford, Connecticut:  On, or about, August 18, 2022, a DeWalt portable table saw, Model DWE7491, serial number 950776, was not used with a self-adjusting hood guard while cutting wood.

29 CFR  1926.404(b)(1)(ii):  On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:  Jobsite located at 131 Beacon Hill Drive, West Hartford, Connecticut:  On, or about, August 18, 2022, ground fault circuit interrupters were not used to protect employees from electric shock hazard when temporary wiring (extension cords) were used to energize equipment.

29 CFR  1904.29(a): A log of all Work-Related Injury and Illnesses (OSHA Form 300) and and the Injury and Illness Report (OSHA Form 301) or equivalent forms were not kept by the establishment:   On, or about, October 20. 2022, the employer did not maintain a Log of Work-Related Injuries and Illnesses (OSHA Form 300) and an Injury and Illness Incident Form (OSHA Form 301).  On August 18, 2022 an employee suffered a finger laceration that involved five days away from work.
