29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. On or about October 17, 2022, and at times prior thereto, employees were walking, sweeping and shoveling dirt around open holes without the holes being covered, exposing employees to trip and fall hazards.
