29 CFR  1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Building 5500: Bay B: Wood Shop:  Employees used a DeWalt Miter Saw that was plugged into a relocatable power tap and not used according to listing and labeling instructions. Employee(s) were exposed to potential electrical shock and/or fire hazard(s).

29 CFR  1910.213(s)(6):  Cleanliness around working machinery was not maintained to prevent fire hazards and ensure the effective functioning of guards:  Building 5500: Bay B: Woodshop:  Employees used a DeWalt Miter Saw that did not have a dust collection system attached resulting in wood dust accumulations on the electrical cords, table, equipment, and relocatable tap located on the floor and the wall outlet.  Employees were potential exposed to fire hazards.
